Application Process
Apply to the network by completing the online application. Do not use these links to add roster staff to your facility or group.
Individuals - Practitioners in a solo practice or wanting a contract in their own name.
Groups - Two or more practitioners rendering services under the same Tax ID on a single agreement/contract for all the practitioners.
Facilities - An organization that provides services - Inpatient, Residential, or Outpatient Programmatic Services.

Application Received
After you submit the online application, you will receive an email from Provider Relations confirming that your application has been received. We will review your application and supplemental documentation. If there are any questions or concerns, Provider Relations will contact you. Once the application is complete, your application will advance to the next step in the process.
Credentialing
You will receive an email notifying that your application has been sent to the Credentialing Team for processing. We will review your CAQH application, licensure, education, malpractice insurance, work history, and other details as applicable, and verify certain elements. If there are any questions or concerns regarding your credentialing information, a Credentialing Coordinator will reach out to you. Once it is complete, your credentialing application will be reviewed by the Credentialing Committee. If your application is denied by the Committee, you will receive a notice regarding the denial and your right to a fair hearing. If your application is approved by the Credentialing Committee, it will be moved to the next stage where you will be sent a contract.
Contracting
You will receive an email from Adobe Sign asking you to review a contract and fee schedule once your credentialing is complete. When you are ready to sign, you can enter your signature electronically and click the “submit” button. A fully executed copy will be returned to you.
Finalizing Contract
You will receive an email from Adobe Sign with a countersigned contract to keep for your records. The countersigned contract will indicate your effective date in our network. Also, your information will be loaded into our systems at this time.
Complete

Minimum Criteria to Join MBH
Maintain an unrestricted license for independent practice
Practice a minimum of fifteen hours per week
Maintain acceptable level of professional liability insurance
Have an email address and secure access to the internet
Able to file claims electronically
Have 24-hour phone coverage
Agree to schedule members within 7 days post-discharge or upon referral

Hold current Board Certification in Psychiatry with
the American Board of Medical Specialties (ABMS) or American Osteopathic Association (AOA
For Addiction Medicine Specialists
Hold Current Board Certification by the American Board of Medical Specialties
(ABMS) or American Osteopathic Association (AOA) AND
Hold one of the following:
Hold current Board Certification in Addiction Medicine by one of the ABMS or
AOA certifying member board; or
Current Board Certification in Addiction Medicine by the
American Board of Addiction Medicine (ABAM);
For Nurse Practitioners/CNS
Certification through American Nurses Credentialing Center
(ANCC) in one of the following;
Nurse Practitioner Psychiatric-Mental Health Across the Lifespan (formerly
known as Family Psychiatric-Mental Health);
